I have seen +5 draws actually occur reasonably often due to perpetuals, when Queens are still on the board. I think we had a discusson here ~6 month ago about such a game of Joker, where CCRL had adjudicated it a loss based on the score, while in fact it was a provable draw. In micro-Max self-play I have seen a +22 draw, which is about the record.

In Gothic Chess SMIRF regularly reports +4 or +5 scores (for itself) after making losing trades, and then indeed continues to lose the game.
